ABOUT Treatment gastrectomy

Gastrectomy in India

What is a Gastrectomy?

A gastrectomy is a complex surgical procedure involving the partial or complete excision of the stomach. The stomach is a muscular digestive pouch responsible for receiving food from the esophagus, secreting gastric acids and digestive enzymes, and churning solids into chyme before delivering it into the duodenum. When the stomach is compromised by malignant tumors, massive ulcers, or uncontrolled hemorrhages, surgical resection is performed, followed by intricate reconstruction of the upper gastrointestinal tract to enable continuous digestion.

Indications for Gastric Resection

A gastrectomy is performed for both oncological diseases and severe benign complications:

  • Gastric Adenocarcinoma (Stomach Cancer): The most common indication globally. Surgery aims for complete R0 resection with clear margins and radical D2 lymphadenectomy to achieve long-term remission.
  • Gastrointestinal Stromal Tumors (GIST): Submucosal mesenchymal neoplasms of the stomach requiring complete wedge or segmental excision with negative margins.
  • Gastric Neuroendocrine Tumors (NETs): Hormone-producing tumors or aggressive atypical carcinoids requiring anatomical gastric resection.
  • Complicated Peptic Ulcer Disease: Refractory gastric or duodenal ulcers presenting with acute perforation, unyielding arterial hemorrhage not controlled by endoscopy, or gastric outlet obstruction from scarring.
  • Prophylactic Total Gastrectomy: Recommended for asymptomatic carriers of hereditary diffuse gastric cancer (HDGC) mutations in the CDH1 gene, who carry an 80% lifetime risk of developing invasive signet-ring cell carcinoma.

Surgical Classifications & Reconstruction Approaches

Gastrectomy is tailored based on tumor location, histology, and disease stage:

  • Subtotal / Distal Gastrectomy: Performed for lesions situated in the lower two-thirds of the stomach (antrum and pylorus). The lower stomach is removed, and digestive continuity is restored using a Billroth I (gastroduodenostomy), Billroth II (gastrojejunostomy), or Roux-en-Y gastrojejunostomy to prevent bile reflux.
  • Total Gastrectomy: Performed for tumors located in the upper stomach, gastric body, or diffuse-type cancers. The entire stomach is excised from the gastroesophageal junction to the pylorus, followed by a Roux-en-Y esophagojejunostomy, where a loop of jejunum is connected directly to the esophagus.
  • Minimally Invasive Laparoscopic & Robotic Gastrectomy: Executed through small abdominal ports, offering 3D high-magnification visualization that facilitates precise D2 lymph node dissection around major vascular structures with reduced blood loss and quicker recovery.

Postoperative Adaptation & Nutritional Management

Inpatient recovery spans 5 to 8 days. Early recovery emphasizes gradual fluid intake and progressive dietary transitions. Because the stomach's storage and mixing functions are altered, patients adopt a "gastrectomy diet" characterized by 5 to 6 small, high-protein, low-refined-sugar meals daily to prevent "dumping syndrome." Routine supplementation with intramuscular Vitamin B12, iron, and calcium is initiated to counter diminished intrinsic factor production.

Treatment Outcomes

  • Primary curative intervention for resectable gastric malignancies when paired with comprehensive D2 lymph node clearance.
  • Definitive elimination of life-threatening gastric perforations, mass bleedings, and mechanical obstructions.
  • Excellent long-term quality of life achievable through specialized dietary counseling and nutritional rehabilitation.
