ABOUT Treatment splenectomy

Splenectomy in India

What is a Splenectomy?

A splenectomy is the surgical removal of the spleen, a highly vascular, fist-sized lymphoid organ situated in the upper left quadrant of the abdomen, tucked beneath the diaphragm and protected by the lower rib cage. The spleen plays key roles in filtering old, deformed, or damaged red blood cells, storing platelets, and orchestrating immune defenses against encapsulated bacterial pathogens. When the spleen is shattered by trauma or involved in severe hematologic disorders, splenectomy is performed to stabilize the patient.

Indications for Spleen Removal

Splenectomy is performed across emergency surgical trauma and planned hematological care:

  • Traumatic Splenic Rupture: Motor vehicle collisions, falls, or direct blows to the left flank causing grade IV or V splenic lacerations with subcapsular hematoma rupture and active intra-abdominal hemorrhage leading to hemorrhagic shock.
  • Immune Thrombocytopenic Purpura (ITP): An autoimmune disorder where the spleen produces antibodies against the patient's platelets and prematurely destroys them. Splenectomy is the standard second-line cure when medical therapies (steroids, IVIG) fail.
  • Hereditary Spherocytosis & Hemolytic Anemias: Genetic red cell membrane defects leading to abnormal spherical erythrocytes that get trapped and destroyed in the spleen, resulting in severe anemia, jaundice, and gallstones.
  • Massive Splenomegaly & Hypersplenism: Extreme spleen enlargement causing severe cytopenias (low red cells, white cells, and platelets) and early abdominal fullness or pain.
  • Hematologic Malignancies & Cysts: Splenic lymphomas, non-parasitic or echinococcal splenic cysts, and primary splenic abscesses refractory to antibiotics.

Surgical Approaches

The choice between minimally invasive and open surgery depends on urgency and spleen dimensions:

  • Laparoscopic Splenectomy (Elective Standard): With the patient positioned in a right lateral decubitus tilt, 3 to 4 ports are inserted beneath the left costal margin. The surgeon divides the splenocolic, gastrosplenic (short gastric vessels), and splenorenal ligaments. The splenic artery and vein at the hilum are safely ligated and divided using endoscopic vascular staplers or ultrasonic energy devices. The excised spleen is placed into a heavy-duty specimen retrieval bag, morcellated, and removed through a port site without large incisions.
  • Open Splenectomy: Conducted through a left subcostal (Kocher) or midline laparotomy incision, essential during emergency trauma resuscitation or when massive splenomegaly (craniocaudal length >20 cm) restricts laparoscopic visualization.

Post-Splenectomy Care & Vaccination Protocols

Because the spleen is vital for clearing encapsulated bacteria, patients undergoing planned splenectomy must receive essential prophylactic vaccinations at least 2 weeks before surgery (or 2 weeks post-surgery in emergency cases):

  • Pneumococcal conjugate and polysaccharide vaccines (Streptococcus pneumoniae).
  • Meningococcal conjugate vaccines (Neisseria meningitidis).
  • Haemophilus influenzae type b (Hib) vaccine.

Lifelong awareness of Overwhelming Post-Splenectomy Infection (OPSI) and immediate medical consultation for fevers are standard protective measures.

Clinical Benefits

  • Rapid, life-saving control of internal abdominal hemorrhage following severe torso trauma.
  • High rate of durable, drug-free platelet remission (>75–85%) in patients with chronic refractory ITP.
  • Resolution of severe hemolytic crises, transfusion dependency, and painful splenomegaly.
