ABOUT Treatment adrenalectomy

Adrenalectomy in India

What is an Adrenalectomy?

An adrenalectomy is the surgical removal of one (unilateral) or both (bilateral) adrenal glands. The adrenal glands are small, triangular endocrine glands positioned atop each kidney deep in the retroperitoneum. Despite their modest size, they produce hormones essential for life, including cortisol (metabolism and stress response), aldosterone (blood pressure and electrolyte balance), adrenaline/noradrenaline (sympathetic nervous system), and androgens. Surgical excision is indicated when hormone-secreting tumors or structural growths threaten cardiovascular and systemic health.

Clinical Indications & Hormonal Disorders Treated

Adrenalectomy is performed for functional (hormone-producing) tumors, non-functional masses, and malignancies:

  • Pheochromocytoma: Chromaffin cell tumors of the adrenal medulla that oversecrete catecholamines (adrenaline and noradrenaline), triggering severe episodic hypertension, pounding headaches, palpitations, and profuse sweating. Preoperative alpha- and beta-blockade is mandatory.
  • Aldosterone-Producing Adenoma (Conn's Syndrome): Benign cortex tumors producing excessive aldosterone, leading to refractory hypertension and profound hypokalemia. Surgery typically cures or significantly improves hypertension.
  • Cortisol-Secreting Adenoma (Cushing's Syndrome): Unregulated cortisol release leading to central obesity, facial rounding (moon face), muscle wasting, hyperglycemia, osteoporosis, and immune suppression.
  • Adrenocortical Carcinoma (ACC): Rare, highly aggressive malignant neoplasms requiring radical en bloc oncological surgical resection.
  • Large Adrenal Incidentalomas: Asymptomatic adrenal masses discovered incidentally on CT/MRI exceeding 4 to 5 cm in diameter, which carry an elevated risk of occult malignancy.
  • Adrenal Metastases: Solitary metastatic deposits from primary lung, breast, or melanoma cancers in carefully selected oligometastatic patients.

Surgical Methodologies

Endocrine surgeons utilize refined minimally invasive techniques for adrenalectomy:

  • Posterior Retroperitoneoscopic Adrenalectomy (PRA): A state-of-the-art technique where the surgeon accesses the adrenal gland directly through three small incisions in the patient's back, bypassing the peritoneal cavity and abdominal organs entirely. PRA reduces postoperative pain, eliminates intra-abdominal adhesions, and facilitates faster recovery.
  • Laparoscopic Transperitoneal Adrenalectomy: The traditional minimally invasive standard, performed through the front abdominal cavity with lateral patient positioning, allowing a wide operative field and straightforward mobilization of adjacent liver or spleen.
  • Robotic-Assisted Adrenalectomy: Offers 3D stereoscopic vision and micro-wristed articulation for dissecting the delicate, short right adrenal vein that empties directly into the inferior vena cava (IVC).
  • Open Adrenalectomy: Employed through a transabdominal or thoracoabdominal incision for large (>10 cm) invasive carcinomas requiring lymph node dissection and en bloc organ resection.

Postoperative Management

Most patients undergoing laparoscopic or PRA adrenalectomy are discharged within 24 to 48 hours. Postoperative care includes continuous blood pressure monitoring and serum electrolyte assessment. Following surgery for Cushing's syndrome or bilateral adrenalectomy, temporary or lifelong corticosteroid replacement therapy is prescribed to manage adrenal insufficiency until the contralateral gland resumes normal endocrine function.

Outcomes

  • Complete biochemical cure and blood pressure normalization in Conn's syndrome and pheochromocytoma.
  • Resolution of hypercortisolemia and reversal of metabolic complications in Cushing's syndrome.
  • Minimal tissue disruption, fast convalescence, and high cosmetic satisfaction with retroperitoneoscopic approaches.
