Ileostomy Creation in India

What is an Ileostomy Creation?

An ileostomy creation is a surgical operation in which an opening is constructed from the ileum (the terminal segment of the small intestine) through the anterior abdominal wall to create a stoma. Because the effluent from the small intestine contains active digestive enzymes and liquid-to-pasty stool, an ileostomy is meticulously constructed as a "spouted" stoma (Brooke ileostomy) protruding approximately 2 to 2.5 cm above the skin surface. This spout directs digestive fluid directly into an ostomy pouch, shielding the sensitive peristomal skin from enzymatic irritation.

Clinical Indications

An ileostomy is performed for protective, curative, or emergency indications:

  • Diverting Loop Ileostomy (Protective): Temporarily constructed following low anterior resection (LAR), coloanal anastomosis, or ileal pouch-anal anastomosis (J-pouch) to divert the fecal stream away from the healing pelvic join, drastically reducing the clinical consequences of an anastomotic leak.
  • Total Proctocolectomy for Ulcerative Colitis: When the entire colon and rectum are surgically removed due to severe refractory colitis, toxic megacolon, or dysplasia, an end ileostomy provides a permanent, curative solution.
  • Familial Adenomatous Polyposis (FAP): Removal of the entire large bowel to prevent inevitable colorectal cancer development.
  • Emergency Colectomy for Colonic Ischemia or Perforation: Toxic colitis, necrotizing enterocolitis, or ischemic bowel gangrene where high-risk primary anastomosis is contraindicated.
  • Complex Crohn's Disease: Refractory perianal Crohn's or severe colonic involvement requiring temporary bowel rest to facilitate medical healing.

Surgical Technique: Loop vs End Ileostomy

Ileostomies are configured according to whether future reversal is scheduled:

  • Loop Ileostomy: A loop of terminal ileum is brought through an opening in the right rectus abdominis muscle, opened transversely, and everted to create a dominant proximal opening for digestive discharge and a smaller distal non-functioning opening. These are typically reversed in a minor procedure 8 to 12 weeks later after imaging confirms complete distal anastomotic healing.
  • End (Brooke) Ileostomy: The terminal ileum is passed through the abdominal wall, everted like a turtleneck sweater over itself to create a 2 cm protrusion, and secured with absorbable sutures to the dermis. This provides reliable permanent diversion.
  • Laparoscopic Execution: Modern surgeons perform ileostomy creation via keyhole surgery, ensuring correct rotational orientation of the mesenteric vascular axis to prevent ischemia or bowel twisting.

Postoperative Management & Fluid Balance

Hospital stay spans 4 to 6 days. Because the colon's water-absorbing capacity is bypassed, initial ileostomy output is liquid and voluminous (typically 600 to 1,200 mL/day). Care teams closely monitor fluid, sodium, and potassium balances to prevent dehydration. Dedicated enterostomal nurses instruct patients on measuring output, applying pectin-based skin barrier wafers, and managing ostomy drainage pouches.

Treatment Outcomes

  • Guarantees protection for deep pelvic colorectal anastomoses, minimizing sepsis risk.
  • Definitive cure for severe, intractable ulcerative colitis following total proctocolectomy.
  • Safe and straightforward reversal for temporary loop configurations once distal healing is verified.
