ABOUT Treatment pancreatectomy

Pancreatectomy in India

What is a Pancreatectomy?

A pancreatectomy is the surgical resection of all or a portion of the pancreas, an elongated organ situated transversely across the posterior abdominal wall behind the stomach. The pancreas serves both exocrine functions (producing digestive enzymes like amylase, lipase, and proteases secreted into the duodenum) and endocrine functions (producing insulin, glucagon, and somatostatin secreted into the bloodstream to regulate glucose). Because of its delicate parenchymal texture, proximity to major mesenteric blood vessels, and caustic digestive secretions, pancreatectomy represents one of the most demanding procedures in general and hepato-pancreato-biliary (HPB) surgery.

Indications for Pancreatic Resection

Pancreatectomy is indicated for benign, premalignant, and malignant pancreatic disorders:

  • Pancreatic Ductal Adenocarcinoma (PDAC): Aggressive malignant tumors arising in the body or tail of the pancreas, where surgical resection offers the sole potential for cure when diagnosed at a resectable stage.
  • Pancreatic Neuroendocrine Tumors (PNETs): Insulinomas, gastrinomas, glucagonomas, and non-functional neuroendocrine neoplasms requiring parenchyma-sparing or anatomical resection.
  • Cystic Neoplasms of the Pancreas: Intraductal Papillary Mucinous Neoplasms (IPMN) with high-risk stigmata, Mucinous Cystic Neoplasms (MCN), and Solid Pseudopapillary Neoplasms (SPN) with malignant potential.
  • Severe Chronic Pancreatitis: Intractable, debilitating abdominal pain resistant to medical therapies, or localized inflammatory masses with ductal strictures and calcifications.
  • Severe Pancreatic Trauma: Major duct disruption or parenchymal transection following blunt or penetrating abdominal injury.

Types of Pancreatectomy Procedures

The surgical strategy depends directly on the anatomical site of pathology:

  • Distal Pancreatectomy: Resection of the body and tail of the pancreas situated to the left of the superior mesenteric vessels. Depending on tumor biology, it may be performed with spleen removal (Distal Pancreatectomy with Splenectomy) for cancer, or as a Spleen-Preserving Distal Pancreatectomy (Kimura or Warshaw technique) for benign lesions. Modern centers perform this using laparoscopic or robotic techniques.
  • Central (Middle) Pancreatectomy: A parenchyma-sparing segmental resection of the pancreatic neck or proximal body for benign/low-grade lesions, preserving both the pancreatic head and tail to maintain exocrine and endocrine function.
  • Total Pancreatectomy: Complete removal of the entire pancreas, duodenum, gallbladder, and spleen, indicated for diffuse multifocal IPMN, hereditary pancreatic cancer, or failed reconstructions. This results in complete insulin-dependent diabetes (brittle diabetes) requiring auto-islet transplantation or meticulous insulin regimens.

Postoperative Care & Management

Hospitalization typically spans 5 to 10 days. Postoperative monitoring focuses on detecting postoperative pancreatic fistula (POPF) via drain fluid amylase levels, preventing intra-abdominal fluid collections, and initiating early ambulation. Patients are prescribed Pancreatic Enzyme Replacement Therapy (PERT) capsules taken with meals to ensure complete digestion and nutrient absorption.

Key Benefits

  • Primary curative modality for localized pancreatic body and tail malignancies.
  • Definitive eradication of premalignant cystic lesions before invasive progression.
  • Substantial pain alleviation and improvement in quality of life for intractable chronic pancreatitis.
