Rectal Resection in India

What is a Rectal Resection?

A rectal resection is the surgical excision of a portion or the entirety of the rectumβ€”the final 12 to 15 centimeters of the large intestine terminating at the anal canal. The rectum is tightly nestled within the bony pelvis, surrounded by autonomic nerves controlling urinary and sexual function, pelvic floor muscles, and vascular bundles. Rectal surgery requires meticulous anatomical dissection along embryological planesβ€”known as Total Mesorectal Excision (TME)β€”to eradicate tumors with clear circumferential margins while safeguarding pelvic nerve pathways.

Clinical Indications

Rectal resection is performed primarily for oncological conditions and advanced benign rectal disorders:

  • Rectal Adenocarcinoma: Malignant tumors of the upper, middle, or lower rectum. Depending on stage, patients often receive neoadjuvant chemoradiation (TNT protocols) followed by radical surgical resection.
  • Large Sessile Polyps: Complex adenomatous polyps not amenable to complete, safe endoluminal endoscopic mucosal resection (EMR/ESD).
  • Refractory Inflammatory Bowel Disease: Severe, chronic ulcerative proctitis or Crohn's proctitis with intractable bleeding, strictures, or malignant transformation.
  • Rectal Prolapse with Redundancy: High-grade full-thickness rectal prolapse treated via rectopexy with resection (Frykman-Goldberg procedure).

Surgical Techniques: Sphincter-Saving vs Non-Sphincter Saving

Colorectal surgeons strive to preserve the anal sphincter and natural bowel continuity whenever oncologically safe:

  • Low Anterior Resection (LAR): Indicated for tumors in the upper and middle rectum. The surgeon resects the rectum with its mesorectum and performs a low colorectal anastomosis using a circular stapler. A temporary protective loop ileostomy is often created to protect the healing low pelvic join.
  • Ultra-Low LAR with Coloanal Anastomosis: For tumors in the lower third of the rectum located close to the pelvic floor. The entire rectum down to the dentate line is excised, and the colon is sutured directly to the top of the anal canal, preserving the external anal sphincter.
  • Abdominoperineal Resection (APR / Miles Operation): Indicated when a lower rectal tumor directly invades the external anal sphincter or levator ani muscles. The surgeon resects the sigmoid colon, rectum, and anal canal en bloc, creating a permanent, comfortable end colostomy on the left lower abdomen.
  • Robotic-Assisted & TaTME (Transanal TME): Innovative platforms that overcome the technical constraints of the narrow male pelvis, providing high-definition 3D visualization and superior nerve-sparing capability.

Recovery & Pelvic Rehabilitation

Inpatient stay averages 4 to 7 days. Patients receive proactive analgesia, early mobilization, and tailored dietary progression. Dedicated stoma nurses provide one-on-one training for managing temporary or permanent stomas. Following low anastomoses, patients participate in pelvic floor physiotherapy and biofeedback to manage anterior resection syndrome (LARS) and restore bowel frequency control.

Clinical Outcomes

  • Gold-standard oncological cure rates with minimal local recurrence (<5%) when TME planes are respected.
  • High rate of sphincter preservation avoiding permanent colostomies in the majority of modern patients.
  • Preservation of bladder and sexual autonomic nerve function through precision robotic visualization.
