ABOUT Treatment fistulotomy

Fistulotomy in India

What is a Fistulotomy?

A fistulotomy is a specialized proctological surgical procedure in which an anal fistula tract (fistula-in-ano) is unroofed, laid open, and cleared of infected granulation tissue. An anal fistula is an abnormal, epithelialized tunnel that connects an infected cryptoglandular gland inside the anal canal (internal opening) to the perianal skin outside (external opening). Because an established fistula tract cannot close spontaneously, a fistulotomy transforms the tunnel into an open groove, enabling it to heal gradually from the bottom up by secondary intention without trapping infection.

Indications & Classification of Anal Fistulas

Surgery is required for chronic discharging fistulas to prevent recurring abscesses, complex branching, and tissue destruction:

  • Intersphincteric & Low Transsphincteric Fistulas: Simple, low-lying tracts traversing only a minimal portion of the internal anal sphincter, where laying open the tract poses negligible risk to fecal continence.
  • Recurrent Perianal Abscesses: Patients experiencing cyclical perianal swelling, throbbing pain, rupture, and purulent discharge linked to an underlying patent fistula tract.
  • Failure of Conservative Dressings: Chronic fistulas causing perianal dermatitis, foul-smelling drainage, and discomfort during sitting or walking.

Surgical Techniques: Sphincter-Preserving Innovations

The surgeon evaluates the precise relationship between the fistula tract and the internal/external anal sphincter muscles using Goodsall's rule, endoanal ultrasound, or pelvic MRI:

  • Classic Lay-Open Fistulotomy: A metallic probe is passed through the tract from external to internal opening. The overlying skin, subcutaneous tissue, and minimal sphincter fibers are divided with electrocautery, the tract curetted, and edges marsupialized to promote clean healing.
  • Seton Placement: For "high" complex fistulas traversing a substantial portion of the external sphincter. A flexible surgical thread or silicone loop (seton) is threaded through the tract. A loose seton keeps the tract open to drain chronic sepsis, while a cutting seton gradually cuts through the sphincter over weeks, allowing fibrosis to maintain muscle continuity.
  • LIFT Procedure (Ligation of Intersphincteric Fistula Tract): A modern sphincter-sparing operation where the intersphincteric space is opened, the fistula tract identified, ligated, and divided without cutting either the internal or external sphincter muscle.
  • Laser Fistula Closure (FiLaC) & VAAFT: Minimally invasive endoscopic/laser treatments delivering radial laser energy to coagulate the epithelialized tract from within, preserving continence entirely.

Postoperative Wound Care & Healing

A fistulotomy is typically performed as a day-care procedure under spinal or short general anesthesia. Postoperative care includes warm water sitz baths 2 to 3 times daily and after every bowel movement to keep the area clean and relax sphincter spasm. Stool softeners are prescribed to prevent straining. The shallow wound heals completely within 4 to 6 weeks.

Key Benefits

  • Highest cure rate among anal fistula therapies (>95% for low fistulas) with extremely low recurrence.
  • Definitive elimination of recurring perianal abscesses and chronic malodorous discharge.
  • Preservation of normal anal continence when executed by experienced colorectal surgeons.
