Duodenal Switch ( BPD / DS ) in India

Duodenal Switch (DS), formally called Biliopancreatic Diversion with Duodenal Switch (BPD/DS), is a type of bariatric (weight-loss) surgery that combines two procedures:

1. Sleeve gastrectomy — removes a large portion of the stomach.

2. Intestinal bypass — reroutes the small intestine so food mixes with bile and pancreatic enzymes closer to the end of the small intestine.

This reduces food intake and nutrient absorption, producing substantial weight loss.  

2. Mechanism of weight loss

BPD/DS works through three main mechanisms:

• Restriction: The smaller stomach limits the amount of food consumed.

• Malabsorption: Shortening the segment where food mixes with digestive enzymes reduces absorption of calories and nutrients, particularly fat.

• Hormonal and metabolic changes: Alterations in gut hormones and nutrient flow can improve satiety and blood glucose regulation.

3. Indications

BPD/DS may be considered for:

• Severe obesity, particularly when substantial weight loss is needed.

• Patients with a high BMI and obesity-related complications.

• Selected patients with difficult-to-control type 2 diabetes.

• Some patients requiring revisional bariatric surgery, depending on their previous procedure and clinical circumstances.
